Live Healthy Lincoln County is a coalition working on community prevention strategies focused particularly on those at high risk for diabetes, heart disease, and stroke. Health system interventions and efforts to link community programs to clinical services is the key goal. This effort is made possible thru The 1422 Public Health Actions to Prevent Chronic Disease Grant from Kansas Department of Health and Environment (KDHE) and Centers for Disease Control (CDC).
RDG Planning and Design helped define the priorities for Lincoln County by creating our
"Healthy & Active Community Strategic Plan"
The objective is to provide meaningful input to the development of a vision and strategic direction that will help many community members understand, relate, and support implementable health projects.
